How to Prevent Dry Eyes from Cataract Surgery

A cataract is when the lens of the eye becomes cloudy. The lens is responsible for focusing light onto the retina. The reason for cloudiness could be due to protein changes in the lens.

Three (3) million Americans have cataract surgery each year.  The Federal government spending $3.4 billion through Medicare to treat cataracts.1 Recent estimates indicate that approximately 15% of the US population older than age 65 has dry eye.2 "If you assume that two million cataract patients are older than 65, and 15% have dry eye, then approximately 300,000 of the patients who undergo cataract surgery each year will suffer from dry eye."   
  
This represents 750,000 people worldwide. 

Why Dry Eyes after Cataract surgery?

Since majority of the cataracts occur in people who are over 65, they are also the group that is most prone to have dry eyes due to:
  • Aging. – Dry eye is a common condition for people over 50.
  • Trauma to the cornea during surgery – creating an uneven surface which causes friction when blinking.  Resulting in inflammation and dry eyes.

Preoperative care.

There are 3 important tests to be done to determine if the patient has dry eyes before surgery.
  1. Rose Bengal Staining,
  2. Schirmer’s Test ( or Zone Quick) to measure tear volume,
  3. Tear Breakup Test to measure tear viscosity.
It would be well advised that patients ask for these tests to be done by their eye doctors before making a decision to have cataract surgery.
If dry eyes are detected before surgery, the recommendation is to treat dry eyes and get the cornea into the best condition it can be to minimize dry eye outcome after surgery.

Post Operative Care:

After cataract surgery the signs and symptoms of dry eyes typically get worse. Following surgery, patients are prescribed a number of eye drops, usually a steroid, antibiotic and an NSAID( pain killer) Prolonged use of postoperative medications may be one of the contributing factors of the patient's dry eye symptoms. In patients with OSD, it is best to stop or taper medications when they are no longer needed.

Hormone Imbalance and Dry Eyes in Women


Menopause Dry Eye affects more than 60% of the middle age women who are either peri-menopausal, in menopause or postmenopausal.

Azithromycin Found To Be More Effective Than Doxycycline In Treating Blepharitis and Meibomian Gland Dysfunction

Azithromycin more effective than Doxycycline for Meibomian Gland Dysfunction and Blepharitis.

Antibiotics are often used systemically to prevent Blepharitis eye infections in dry eye patients.  Blepharitis and Meibomian Gland Dysfunction often go hand in hand.


There are several studies concerning the use of topical azithromycin in ocular surface diseases. . Dr. Gary Foulks and his group at the University of Louisville compared the effectiveness of topical azithromycin versus oral doxycycline therapy in meibomian gland dysfunction and Belpharitis.

Twenty-two subjects were treated with topical azithromucin solution for one month and seven subjects were treated with oral doxycycline for two months. The study concluded that while both topical azithromycin and oral doxycycline improved clinical signs and symptoms of meibomian gland dysfunction, the "response to azithromycin is more rapid and more robust than doxycycline."
Yet another study from the Ohio State University School of Optometry evaluated the efficacy of a four-week treatment with topical 1.0% azithromycin solution versus rewetting drops in patients with contact lens related dry eye. An over two-hour improvement in comfortable contact lens wear time was noted throughout the four-week study period with azithromycin solution use.

Why Success for Treating Dry Eyes Are Low in Doctor's Offices.


A recent “Industry News” article in the AOA News  reports several very interesting facts indicating the low rates of success in treating dry eyes in doctor's offices.

These facts include:
1) Nearly half of all U.S. adults (48%) experience one or more dry eye symptom(s) regularly;
2) 19% of women age 55 and older have experienced dry eye symptoms for more than 10 years,
3) 69% who experience one or more dry eye symptom(s) have not visited a eye care professional to treat symptoms
4) of those who visited an eye care professional to treat their dry eye symptoms, 19% visited more than once before finding relief, and 22% reported that they still have not found relief.
This market research indicates that dry eye complaints is more than any other ocular condition in a doctor's office. Rheumatoid Arthritis with Dry Eye Syndrome- Relief in One Week!

This is my story.  I want to share with people who has struggled with the same issues and seek help.

I have Rheumatoid Arthritis, which is one of the autoimmune disorders. My dry eye problems began last year, I started to notice that when I was driving I was blinking a lot because the light was bothering my eyes. We also moved to the mountains and it is very dry due to high altitude. This further aggravated my dry eyes. In the morning my eyes were stuck with gooey stuff. I could not read for long or watch T.V. because of the constant blinking. I also felt something were in my right eye, I could not wear eye makeup because of the irritation.

I went to my doctor, he told me to run a vaporizer and use over the counter eye drops, none of this helped. I had also heard from friends who had gone to their eye doctors for help and weren't helped very much. One was given a supply of prescription eye drop, but it made her eyes feel worse.

Tips for Computer Vision Syndrom Relief from NIH

For those who are suffering from Computer Vision Syndrome.  Your eyes are tired, can't focus and getting blurry vision.  You might be interested in this abstract from the National Institutes of Healt.

Computer vision syndrome (CVS) is a real problem for people who work in front of a computer screen all day long.  This often lead to chronic dry eye syndrome.  The symptoms of this dry eye condition includes headaches, blurred vision, neck pain, fatigue, eye strain, dry, irritated eyes, and difficulty refocusing the eyes. These symptoms can be made worse by improper lighting ie. bright fluorescent overhead lighting or glare,  or air moving past the eyes and face e.g. overhead vents, direct air from a fan.

The normal blink rate in human eyes is 16–20 per minute. Studies have shown for those working on computers, the blink rate decreases to 6–8 blinks/minute.  This leads to chronic dry eyes.    The long hours required for focusing the eyes puts strain on the ciliary muscles of the eye.

Tips on how you can help
Sometimes, rearranging your work place environment to create better ergonomics can help to reduce eye strain related to CVS.  A good pair of computer eyeglasses can help you see better at just the right distance range needed for viewing a computer screen.

Eye drops don't always help!

Most patients, however, reach for over the counter artificial tears, which washes away the thickening agents in the tear and make the tear easier to evaporate. The eyes feel even drier.
